Advance care planning seminar offered by St. Paul Elder Services, Fox Valley Advance Care Planning Partnership

St. Paul Elder Services (SPES), a community dedicated to enriching the life experience of the individuals they serve, is partnering with Fox Valley Advance Care Planning Partnership to offer a free seminar on advance care planning. The session will help participants explore how they want their healthcare handled, how to have important conversations about their wishes and complete documents that will help guide the future.  

SPES and Fox Valley Advance Care Planning Partnership have a collaborative relationship to help people prepare for the next chapter of their lives, and in turn, help family members navigate the future knowing their loved ones’ wishes in full.  

ACP & ME – Getting Started: Advance Care Planning will be held on Monday, November 14 from 5:30 – 7:00 p.m. at the Hoffman Memory Care Resource Center at St. Paul Elder Services in Kaukauna. The entrance is located at 221 E 13th Street (map).  

This event is free, but participants must register by calling (920)766-6020 ext. 102 or emailing lauriel1@stpaulelders.org.  Free forms and a boxed meal will be included for all attendees.
