Life Enrichment Center

The Life Enrichment Center, located at the St. Paul Elder Services Kaukauna campus, is a resource for individuals seeking support to live independently in the place they call home for as long as possible. It is home to Club Gabriel, a Passages program, and is where St. Paul at Home services are coordinated.

Club Gabriel

Club Gabriel is a state-certified adult day program specializing in memory care. We serve community members 55 and older who desire engagement and companionship. Care partners that need respite to work, run errands or take time for self-care can bring their loved ones to us for a day full of engagement, healthy meals, and opportunities for social interaction.

Our Adult Day Program offers therapeutic activities for those individuals who are still living in their own homes. Operating Monday through Friday, this program includes meals, activities, and socialization opportunities to keep the person engaged and active in order to remain in their own home for as long as possible. Club Gabe is a covered benefit under contract with the Veteran’s Administration.

St. Paul at Home

St. Paul At Home is an affordable alternative for our senior community members who desire to remain independent in the place they call home for as long as possible. This independence also can be achieved by those living with memory loss, as our team specializes in providing care for those with memory care needs. We are dedicated to serving our community by empowering individuals to enjoy a better quality of life on their own terms. We are committed to providing products and services to promote responsible, enjoyable aging by preventing premature long-term care, relieving isolation, and improving overall quality of life.

Our St. Paul At Home manager, specializing in senior and memory care services, will complete a comprehensive in-home assessment. A personalized plan will then be developed with the client and their care partners, identifying needs and implementing the services right for them.

Remote Monitoring and Support Platform

This touchscreen device delivers cognitive assists in the form of reminders for daily living activities, medication, calendar appointments, and much more. This system provides families with a greater sense of security and allows them to maintain close communication with their loved one while they retain their independence at home.

Personal Emergency Response System (PERS)

24/7/365 Connection to an Emergency Operator

This device is for unexpected emergencies. Features include water-resistant buttons, built-in fall detection and two-way voice communication. The equipment is compact and lightweight and has range of up to 800 feet from the base unit. Battery life can range from 60 hours to five days.

MedMinder

Medication Management

Hassle-free medication management for caregivers and patients alike with this remote locking pillbox that is intuitive and user friendly. Find peace of mind with customizable alerts to take the right medication at the right time. MedMinder also offers automatic prescription refills, mailed medication in organized packaging and monthly check-in calls.

Companionship and Assistance

Our professional associates are available to provide companionship and assistance in our clients’ homes with light housekeeping, bathing, dressing, laundry, light meal preparation, errands, and more. We provide specialized care for clients with memory loss.

Meals on Wheels

Meal Delivery Service

Volunteers deliver nutritious meals to our clients, promoting independence and opportunities for socialization. Meals are delivered Monday through Friday, and clients are able to choose the frequency of delivery.

Foot Clinic

Foot Health Service

This weekly program derives its name from the wonderful way our clients feel after having attended the clinic. Specially trained registered nurses assess and treat general overall foot conditions, nails, and calluses.

Transportation Assistance

Wheelchair-accessible Transportation

Transportation is available to our clients for medical appointments, shopping, and other social events. Our vans are wheelchair accessible.
