In honor of National Healthcare Decision Day, St. Paul Elder Services is partnering with Fox Valley Advance Care Planning Partnership to offer a free seminar on advance care planning. Celebrated on April 16, National Healthcare Decision Day exists to educate and empower the public and providers about the importance of advance care planning.
ACP & ME – Getting Started: Advance Care Planning will be held on Thursday, April 20 from 4:00 – 5:30 p.m. at the Hoffman Memory Care Resource Center at St. Paul Elder Services in Kaukauna. The entrance is located at 221 E 13th Street (map).
This event is free, but participants must register by calling (920)766-6020 ext. 102 or emailing lauriel1@stpaulelders.org. Free forms and a boxed meal will be included for all attendees.
