Partnering for Advance Care Planning

St. Paul Elder Services (SPES) and Fox Valley Advance Care Planning Partnership (FVACPP) have come together to advocate for the significance of advance care planning. Through this partnership, they aim to raise awareness about the importance of making healthcare decisions early by providing easy-to-understand resources and support to the community.

Advance care planning enables individuals to make informed decisions about their healthcare options if they are unable to express their wishes later in life or due to unforeseen circumstances.

As a healthcare provider, SPES understands how is essential it is for people to have a plan, known as an advance directive, in place to ensure that their medical wishes are respected. Yet, despite the critical importance of advance care planning, many individuals remain unaware of the process or fail to engage in it.

Fox Valley Advance Care Planning Partnership is a collective of healthcare providers and organizations dedicated to promoting advance care planning discussions within the community. Through its partnership with SPES, the two organizations offer various resources and programs, including community engagement events, to educate people on the importance of advance care planning and guide them through the process.

This partnership represents a vital step in promoting access to the essential tools individuals need to make informed decisions about their medical care and emphasizes that advance care planning is not just for individuals nearing the end of their lives.

By promoting advance care planning, these organizations hope to encourage people to have conversations with their loved ones and healthcare providers about their preferences, values and future health care wishes.
