Whether at home, in the community, or on our inviting Kaukauna and Green Bay campuses, Passages offers compassionate, personalized care and meaningful engagement for every stage of life.

✨ The Passages Continuum of Care — At St. Paul Elder Services, our Passages program thoughtfully supports individuals living with dementia and their families, offering personalized care and engagement at every stage.
Community Services (day programs for older adults living at home or in the community, providing dementia-specific support, engaging activities, and compassionate care—while giving caregivers valuable time to rest and recharge):
✅ Memory Connections — Brain health enhancement for early memory loss.
✅ Memory Reflections — Daytime respite and engagement for moderate memory needs.
✅ Club Gabriel — State-certified adult day program promoting socialization and enrichment.
Residential Memory Care:
✅ Memory Care Assisted Living — Residential care for individuals who need a safe, supportive environment while maintaining as much independence as possible. (St. Paul Manor on our Kaukauna Campus and St. Francis Place on our Green Bay Campus)
✅ Long-Term Nursing Home Care — Nurturing, individualized care for people living with Alzheimer’s disease and other dementias, offered in two distinct, home-like neighborhoods that honor each person’s story and stage of life.
- Appletree Court: This inviting neighborhood features 18 private rooms, two safe courtyards, an open kitchen, and a cozy lounge—creating familiar, comforting spaces where residents can continue daily routines and enjoy group activities in a safe, structured setting.
- Poppy Path: Also offering 18 private rooms, along with its own dining and lounge areas and a safe courtyard, Poppy Path supports individuals whose changing needs require more hands-on care and supervision. Here, residents receive attentive nursing care while continuing to enjoy social opportunities and recreation tailored to their abilities and interests.
✅ Hospice & Palliative Care — Compassionate, Medicare-certified care focused on comfort, dignity, and peace at the end of life.
